It looks messy but on closer examination this layout offers very rapid access to the high pressure reel, both hose carts, the pump plumbing and controls, the ladders and the doors to the stretcher compartment too.

Not obvious in this photo is the right side door to the medical compartment, a separate entry for the medical attendant to accompany the patient. This is for the crew member who attends to the patient, but otherwise rides in the crew cab.
