Like most ICUs, this is based on a bus body, but in this case with relatively few modifications. The large number of sliding windows suggests that air conditioning was not a feature, which is rather unfortunate in Mumbai's climate!
The big old brass bell is very noticeable, and the red and white checkers around the roof are, of course, directly derived from the UK practice for identifying command units...
